Cardinal Health (CAH) Analyst Ratings
Bulls say
Cardinal Health is well positioned because its dominant U.S. pharmaceutical distribution franchise sits inside a highly concentrated industry, where scale, 70,000-plus daily deliveries, and 99.8% on-time performance create durable barriers to entry and recurring volume growth. Its bullish case is strengthened by a deliberate shift toward higher-margin services and specialty platforms, including Sonexus, MSO, and nuclear medicine, with the Other segment expected to become ~25% of total operating income by 2028 and fiscal 2025 revenue of $222.6 billion supporting robust cash generation. Management’s 12% to 14% adjusted EPS CAGR outlook from fiscal 2026 to fiscal 2028, together with at least $2.25 billion of planned share repurchases, suggests disciplined capital allocation should amplify earnings as specialty, biosimilar, and precision-medicine demand expands.
Bears say
Cardinal Health is viewed negatively because its core Pharmaceutical and Specialty Distribution business depends on razor-thin gross margins, while drug pricing pressure, biosimilar mix shifts, and alternate fulfillment models could slow revenue to mid-single digits and cap margin expansion near 1.8%. The company also faces meaningful execution risk in higher-growth areas such as Nuclear & Precision Health Solutions and Sonexus, where FDA approvals, radioisotope supply, automation, and regulatory compliance are critical to sustaining the hoped-for 15-17% and 5-7% profit growth trajectories. Added pressure comes from customer concentration, tariff and input-cost inflation, and the fact that fiscal 2025 sales to the five largest customers were about 43% of total sales, making Cardinal especially vulnerable to pricing concessions and contract changes.
